ATHLETIC REGISTRATION
TMMS has transitioned all Athletic Registration to the Intouch online portal. One location for signing up for athletics along with the ability to make payments for students’ fees and fines.
Attention: if you qualify for the free or reduced lunch program, you need to complete the lunch application & Consent to Share Form (every year) by printing and returning the form to Child Nutrition Services (available on district website under district departments, Nutrition Services, Free Reduced Program). If you are eligible, do not pay the fees for the ASB card or the Athletic Fee while registering for sports as these will not be refunded once processed.

ATHLETIC FORMS/SPORTS CLEARANCE CHECKLIST
Before students may participate in a sport, they must have completed the following forms online. Hardcopies will not be accepted, all forms need to be completed online.
- Athletics & Activities Health and Emergency Authorization Form (TMMS)
- Athletics & Activities Participation Form with Athletic Code (TMMS)
- Current physical form uploaded - good for 24 months from date of exam
- Concussion and Sudden Cardiac Arrest Awareness Form (TMMS)
- Current Medical Insurance
- ASB card for current school year ($25 or $50 enhanced)
- Yearlong Athletic User Fee paid ($65 this fee is non-refundable after the first competition of the sports season.)
- An additional fee for Football ($45 this fee is non-refundable after equipment is distributed.)
